A Health Risk Assessment is required for all patients/clients who want to see Dr. Heuser for a consultation and nutrition education. Many patients can use their health insurance for this service if you have Anthem, Humana and United Health Care. The information you submit will capture your personal and family medical and surgical history as well as the medicines you are taking.
